Please remember that the health care providers you used under your prior health plan may not be in-network providers under the health plans offered by Community. This means that if you choose to continue receiving services from non-network providers, Community health plans may pay benefits at a lower level than you received under your prior plan.

You may want to apply for continuity of care through the insurance company of your new plan.
